I want to build a controller for my modal in Laravel

Hey guys, i'm new on Laravel and i'm trying to implement a modal where the user sends a clasification and comment about the service he received. The thing is, i'm a bit confused about the structure of the controller, cause the data has to be sent to a database table that brings selected values from other table, plus the comment the user is writing. Can anybody recommend a video about that?
