What do you mean with the custom ip address field. What don't you understand about it?
The IP address should be the IP connected to the VPS server.

Guide to setup custom VPS server on forge.
I was trying to create a Custom VPS on laravel forge but stuck in the ip address field. can anyone help me out to setup the forge custom VPS.

Little confusing to be honest. You'd of course get the IP from your server provider. Have you watched the Laracasts series about Forge - https://laracasts.com/series/learn-laravel-forge ?
