Integrating Nova fields into a custom tool

Hi there, I'm wondering if there's a way to integrate Nova fields into a custom tool. I created a custom Tool in order to make a different form than the one in the resource. But I would like to use the fields that are already created in Nova, but I don't know how to do that, or even if it's possible to do that. Anybody has an answer for that?
