Card and envelope templates

I need to create a report for my user to print on two different non-traditional formats: 3x5 card size and envelope size for a 'set' of people (different instances). Does anyone know a place where I can see examples, etc., for sizing the pages appropriately for these kinds of 'reports'? I have imagined it in pdf format, but I am open to web pages that are easily printable, etc.
