Generate Table of Contents using dompdf/dompdf library

I want to generate a table of contents using dompdf library. I have found this code and it seems not to function as expected.

When I preview the PDF, it still shows the placeholders %%CH1%% instead of actual page numbers.

Has someone encountered this problem before?

It looks like dompdf doesn't have support for Hoc out of the box as per this issue

It looks like there might be a hacky CSS workaround but depending on how much of a pain it would be go change you might consider mpdf as it has this feature.
